There are different codes for screening and for screening that becomes diagnostic.

Also different sets of codes for insurance payers vs Medicare.

As part of the Affordable Care Act (ACA), Medicare and third-party payers are required to cover services given an A or B rating by the U.S. Preventive Services Task Force (USPSTF) without a co-pay or deductible. That is, the patient has no patient due amount.

It's good to note that Medicare still waves the deductible if screening converts to diagnostic or therapeutic. I don't know about other insurance payers.
