Didn't we used to put commas before "and" in a series of three and not put a comma in a series with "or"?
Old way: awake, alert, and oriented.
Now: awake, alert and oriented.
Old way: No nausea, vomiting or diarrhea.
Now: No nausea, vomiting, or diarrhea.
