It is only a letter from some firm of solicitors in Belfast, it's not a judicial request. As Martyn said you do not have to fill it in, they can't demand you do what they are asking. I can't remember who I was with this time last week let alone sometime 47 years ago. Signing for a letter does not mean that you have to comply it means that the letter was delivered that's all. If you have information that must be handed over then some one would knock at your door, I mean that what the police do, they don't send letters asking for the information. Any way they should go to the channels that hold that information, MOD etc and not Mr Davis or Mr Mills or Mr who ever was serving in Ulster in the 1970's.
